As a Social Worker with our Health Navigation Team, you will support individuals and families facing complex health and social challenges through assessment, care planning, advocacy, system navigation, and coordinated care. Working with an interdisciplinary team, you will connect clients with resources, strengthen self-management skills, and help reduce barriers to health and well-being.
This role is an opportunity to contribute to a community-centred model of care that recognizes the connection between social circumstances and health outcomes. We are seeking a skilled and compassionate Social Worker committed to social justice, cultural humility, and collaborative practice.
Rutland Community Clinic (RCC) is a Community Health Centre focused on building trust in healthcare and improving access for equity-deserving communities. We primarily serve Indigenous and Newcomer populations through an integrated, team-based primary care model.

The Clinic is governed by the Okanagan Community Health Society (OCHS), a non-profit society led by a volunteer board of community members.
Education, Training and Experience:
Master’s or Bachelor’s Degree in Social Work from an accredited university.
Two years of recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or an equivalent combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers.
Valid BC Driver’s License.
